Yes I believe it's hardware issues.
You could update your phone with Seus and see if that helps, or reset the phone, or flash it which you have done but also use get some cam drivers.

Here are many camdrivers for K750 / W800 / W810: Camdrivers - main :)
Sarmad 5.8 is recommended by many. Haven't tried it yet.
