Try to reflash the MAIN firmware(but only the MAIN), and try again. Also you can try the jdflasher to apply the patches instead of SETool2 Lite.

If the padlocks are open, then your phone is unlocked.

The QA patch is recommended, if you want to apply other patches in future. Otherwise you can skip it if you want...

cheers simox reflashed the phone i was using a cable for a k750i that i borrowed, but managed to unlock it with the normal usb lead that come with the phone.

It is an absolutely normal behavior, because we patched the MAIN firmware to trick the SIM-lock checker functions, but the flashing/patching tools read the SIM-lock status from GDFS, what is untouched.

If your main is same like 1 in phone you don't need FS or CUSTPACK,but if isn't than you need all 3 files and first do full flash and than apply MAIN+QA patch
