Request access to download area Error

When i request access to the download section. after clicking send, all i see is a white space. can anyone help me with this:

how do you know if it's been sent or not. I am having the exact same problem.

If You Recieve The Message Like Above It Means That Your Request Has Been Sent.

Otherwise contact a admin ( Top Sony ).
