unlocking my w580i

I've recieved the unlocking code for my phone but i havent got a clue how i type them all in as all my phone says is insert sim

press <**< then will take u to the menu then type it in there are 5 locks pick the one that is locked and type the code in then it wil unlock it what phone and were did u get the code
